Prepare an envelope mailing to USCIS - lower right corner, clearly mark Special Student Relief.

 

• completed Form I-765 with check or money order payable to USCIS for $340, or affidavit if you are requesting a waiver (place this on top of your application)

 

• copy of the I-20 with a recommendation for employment from the college International Education Office to SEVIS

 

• a letter explaining your reasons for asking for employment off-campus and reasons for your inability to find no-campus employment (perhaps include supporting documentation)

 

• a photocopy of passport identity page (photo, number, and expiration date)

 

• a photocopy of both sides of your I-94

 

• two color photos with white background.  Lightly print your name on the back of each photo with pencil (if you have an "A" number, add that also)  The photos must be taken within 30 days:

     in color, but on a white background

     not mounted

     printed on thin, glossy paper

     show a three-quarter front profile of the right side of the face. Right ear must be visible unless you are wearing a headdress as required by your religion

     proper measurements = total size 1.5 inches by 1.5 inches

    distance from top of head to just below chin 1.25 inches

 

• photocopies of driver’s license (if you have one) to establish identity

 

• photocopies of any previously issued EAD cards
